If a UTI-like presentation involves urethral discharge or follows unprotected sexual contact, chlamydia should be considered and a single 1g dose of azithromycin is appropriate. For classic UTI caused by E. coli, nitrofurantoin or trimethoprim-sulfamethoxazole are the first-line choices.

Alcohol does not directly reduce the effectiveness of azithromycin. However, both alcohol and azithromycin can cause nausea and gastrointestinal upset — combining them worsens GI side effects. More importantly, alcohol can impair the immune system's ability to fight infection, potentially slowing recovery.
